Some practices that take care of pediatric patients have the privilege of having that child need an adult
care practitioner. And inside of their own practice, there may be our adult doctors who can take adult
caring doctors, internists, family physicians, and other healthcare providers who can help take care of
that pediatric patient. this is a short introduction to a resource that I want you folks to, download and look
at. It’s called the Six Core Elements of Healthcare Transition and Implementation Guide, transitioning to
an Adult Approach Healthcare without changing, clinicians or practices. And from that standpoint, it
goes through the six core elements of transition, dealing with policy tracking and monitoring, transition
readiness, transition planning, transitioning to an adult approach to care, and then ongoing care. And it
walks through some steps for how to get going and using this implementation guide. And so I please ask
you to download this guide and work through a nine step process that the guide has.
It’ll take you about half an hour to an hour to do it. but I think it is very, valuable resource. It was funded
by HRSA and is available at the GOT transition website, but also in the tab just above that says Materials,

congratulate you for getting that pediatric patient successfully through youth and teenage years into an
