My fellow healthcare practitioners, it is a challenge to transition from one doctor to another, let alone from a pediatrician where your, the parents are responsible for the decisions with the pediatrician to empowering the child, now the new young adult, to be the decision maker in consort with their health workers, their community support system.

Please be patient as you work with that, young adult as they emerge in this practice. Mentor them and guide them. But also please take the time to do the advanced courses on transition of care. the documents from GotTransitions.org of a wonderful HRSA program are very appropriate for this, use and will make it easy for your practice to do a good job.
