Come join us this Saturday for some fun, a free lunch, and an opportunity to learn about the basics of transitioning from pediatric to adult services.

We’re excited to announce a new FREE educational series for transition age teens (14-18 years of age). Our goal with this program is to start the education of our teens and young adults for the eventual shift to adult health care and provide them with a “starter kit” to do it with confidence and knowledge when the time comes.
